CYP Referrals
Our Universal Referral Form is designed to make accessing therapeutic support simple, inclusive, and flexible for all families.
The form can be completed by anyone supporting the child or young person, including:
Parents or carers
Schools and education staff
Early Help practitioners
Social workers and Children’s Services professionals
Other involved professionals
We encourage the child or young person’s voice to be included wherever possible.
